Eligibility Criteria for West Bengal Accredited Social Health Activist Recruitment 2020
| Should be a Married/Divorced/Widow woman |
| Should be a permanent resident of the same area under the health Sub Centre for which she is applying |
| Should be a Madhyamik of equivalent examination passed or appeared. Candidates possessing higher qualifications are also eligible. |
| Preference will be given to Grade I & Grade II SHG Members/ Trained Dais/Link workers keeping the selection criteria fixed. |
Age Criteria:

| Interested candidates should fill up prescribed form (Annexure A) in her own hand writing with blue/black pen and enclose the following: |
| Birth Certificate or 10th class certificate |
| Voter or Ration card as proof of residence and Aadhar Card |
| Self attested copy of Marriage Registration Certificate in case of Married woman or self attested copy of decree of divorce in case of divorced women or self attested photocopy death certificate of husband in case of window. |
| Two copies of identical passport size colored photo, one for application form and another for admit card |
| An envelope with postal stamp of Rs 5/- affixed and full postal address of the candidate written on |
| The envelope containing the application form and enclosures should be superscripted as” APPLICATION FOR ENGAGEMENT AS ASHA, name of Sub Centre applied for________ and name of area_________” and should be addressed to and to be submitted in the drop box kept at the Concerned block development office or via speed post or Registered post to their respective Block Development office. |
